The sole objective of the investigation into this accident is for the prevention of future accidents through the ascertainment of its causes and circumstances. It is not the purpose of the investigation to determine liability nor, except so far as is necessary to achieve its objective to apportion blame.

This report is not written with litigation in mind and shall be inadmissible in any judicial proceedings whose purpose, or one of whose purposes is to attribute or apportion liability or blame.

This report should not be considered as a full casualty investigation in accordance with IMO MSC 255(86), however it aims to highlight the pertinent aspects of this incident with the objective of preventing re-occurrence.

It is understood that the vessels operator is conducting their own investigation and the Flag State of the vessel has been informed.
