This is the Declaration of Maritime Labour Compliance Part I. This document is prepared by the flag State and outlines each element of the flag State’s laws that gives effect to the Maritime Labour Convention 2006. It is the reference source for inspectors both for certification and inspection. This Declaration and the associated Part II, completed by the shipowner, must be assessed together as a part of the overall process for MLC certification.

Application for Minimum Safe Manning Document

This form should be completed by any owner or manager on applying for a Minimum Safe Manning Document as required by Regulation 14 of Chapter V of SOLAS

CSR Form 2- change of CSR details

This form is to be used when any of the details contained in the ship’s CSR are to be changed. All the data fields should be completed, but fields which are not changed should be entered as N/C. One copy of the Form 2 is to be retained on the ship’s CSR file on board, the other is to be sent to ADOMS. On receipt of the completed Form 2 ADOMS will issue the new CSR and when this is received on board it should be signed and dated by the Master and added to the ship’s CSR file. The Form 2 should be retained on the file as part of the permanent record.

This checklist should be filled in and submitted with any application for seafarer’s documents including endorsements and seafarer’s books.

Declaration of Familiarisation for Senior Officers

This Declaration is required to be signed by all senior officers as a part of their application for an Antigua and Barbuda endorsement. The Declaration confirms that the applicant is familiar with the Senior Officer’s Handbook which sets out the essential aspects of Antigua and Barbuda law and practice that senior officers need to be familiar with.

CSR Form 3 (Index of changes) blank form

This form is to be used to record changes to the ship’s current CSR. The changes should all be listed and the Form 3 listing them inserted in the CSR file where it is to remain permanently. On recording changes on this Form, the Company or the Master should also complete a Form 2 setting out the change and place one copy of the form 2 in the CSR file as well. Another copy of the Form 2 should be sent to ADOMS as quickly as possible for the preparation of the next original CSR.

This is the Declaration of Maritime Labour Compliance Part II. This document needs to be completed by the shipowner showing how his systems and arrangements on board are structured to comply with each element of the Maritime Labour Convention 2006 that is listed on the Document. This Declaration and the associated Part I, issued by ADOMS, need to be assessed together as a part of the overall process for MLC certification.

This is an updated version to replace the previous version which was reported to have some problems with formatting. As it is the content which is important, DMLC Part II ‘s complated on the old format are still acceptable, but this current version produces a better appearing document.
